Care Pathways for Outpatient Referrals and Emergency Admissions Indicators The GPC has now clarified whether the care pathways for Outpatient Referrals and Emergency Admissions indicators (QP6-QP11) have to be newly developed or whether they can be ones currently in development at the time the indicators were published? The joint GPC/NHS Employers Supplementary Quality and Productivity Guidance, states that: The QOF guidance/SFE is clear that the pathways to be developed should be new. However, where a pathway is still in the development stage and allows the opportunity for practices to engage in development then, subject to agreement between the PCT and practice, this would be acceptable. The wording of indicators QP8 & QP11 in the SFE stating that  The practice engages with the development of and follows three agreed pathways does not necessarily imply a completely new pathway. The intention is to give practices a say in what pathways are developed and what they finally look like and to avoid unacceptable ones imposed upon them - at the same time giving some flexibility to use acceptable ones that are being developed but not yet fully implemented. The SFE and supplementary guidance were designed to prevent PCO imposition of pathways whilst allowing GP groups to use whichever pathway they believe to be the most appropriate. There is therefore some flexibility for practices to use developing pathways already in existence as long as they are in the development phase as far as the practice is involved. What you cannot do is to designate a pathway you already use as a  new pathway for the purpose of QOF. GP Certification of Missed Exams Thank you Suffolk LMC for bringing to our attention the latest guidance on the (non) provision of sick notes for students missing exams. If you are asked to provide a note for a student who has missed an exam tell whoever is asking that this is not GP work and that the Office of Qualifications & Examinations Regulator (Ofqual) has agreed with the BMA that GP Certification for missed exams is not necessary. To quote from the correspondence which states this please go to the LMC website www.norfolkwaveneylmc.org.uk. Transporting Patients to and from Home to Care Homes/Community Hospitals With the growing emphasis on admission avoidance practices are looking at alternatives to calling 999 and admitting patients, very often unavoidably, to acute beds. We are, however, aware of instances where practices have agreed with their community/social services colleagues that a patient's needs would be best met in a care home or a community hospital setting and a suitable bed has been identified, only to be told by the Ambulance Trust that it is currently not contracted to transport these, often very frail, patients. Trawling around the local health economy to establish who can authorise this transport is often not an option when admission is urgent and we know of instances where, rather than delay the transfer any further, practices have agreed to the Ambulance Trust billing them for the journey. The practice has then forwarded the invoice to the PCT for payment (probably with its corporate fingers crossed!) This may well remain the most pragmatic way of resolving a silly situation but we have been advised of an alternative route by Ms Helen Izatt, NHS Norfolk Contracts Manager, Tertiary & Special Cases (01603 257044), that: "A Patient Transport & Clinical Assessment and Advice Service is being developed when there will be one central access point. In the meantime NCH&C is managing transport on behalf of NHS Norfolk and the contact point is Wendy Halsey or Carol Lendrum at Elliott House". If GtY&W practices have experienced similar difficulties perhaps they would let us know. CQC Registration The Department has launched a consultation on delaying the start date of CQC registration for GP practices to April 2013. LMCs and Practices have the opportunity to comment on the consultation, which available at: www.dh.gov.uk/en/Consultations/Liveconsultations/DH_127174 .
